There is a 4 step process for completion:

  1. Personal Information Form

  2. Online DBS check, involving completion of an online form and an ID check

  3. Reference request

  4. Read Safeguarding policy and confirmation


Step 1: Personal Information Form

To complete the personal information for children, youth or vulnerable adult work, please click here.


Step 2: Online DBS Check

To proceed with the online DBS check for Severn Vineyard, please click on the link below to access the system.

You will have been given a job title, something along the lines of “Children’s Church Worker” or “Vulnerable Adults Befriender”. If you are unsure of what you should put in the position field, please go back to the person who asked you to complete this.

When this is done, please email email  safeguarding@severnvineyard.org to inform us that you have submitted the application. We will then arrange a time to check your ID.


Step 3: Reference Request

This step is to be completed by the named person on your personal information form. Please click here if you have been asked to act as a reference for someone at Severn Vineyard. There is a form for you to complete on that link.

Step 4: Safeguarding Policy
